We want to make sure each visit to PTF is pleasant, productive, and worry-free.

Your first visit

Please plan to arrive 15 minutes before the start of your first appointment to complete your new patient paperwork.  This paperwork will include information about your injury, current functional limitations, and medical history.  For your first visit, be sure to:

  • Bring your insurance card or insurance information
  • Bring your photo identification card
  • Wear comfortable clothes
  • If your physician requested physical therapy, please bring referral or prescription

Your physical therapy treatment

For every appointment at PTF, you will be seen one-on-one by a licensed physical therapist.  On your first visit, your physical therapist will evaluate your condition and identify the source of your pain or physical injury. Your physical therapy initial evaluation will include a thorough examination of muscle, nerve, tendon, ligament, bone, and body tissues/organs as they pertain to your physical complaints or limitations.  If a physician previously examined you, than your initial physical therapy examination will be directed on the specific region or condition identified by your doctor.

After your physical therapy evaluation, your therapist will develop a treatment program that will often address physical deficits, such as: strength, posture, flexibility, neurological status, joint motion, and functional limitation with activities of daily living.  Physical therapy treatment may include the use of manual therapy techniques and exercises.  Therapeutic modalities may be used to decreased swelling, decrease pain and/or soothe injured muscles.  You will also receive instruction in a home exercise program.  The home exercise program may include: education on proper body mechanics, posture to prevent re-injury, exercise instruction, cold/heat therapies and general self-care. Your therapist will plan, implement, and monitor your progress throughout the treatment program.

The initial evaluation will typically last approximately 60-75 minutes.  Subsequent appointments are scheduled for approximately 60 minutes.  Please arrive 5 minutes before each appointment.  If you are unable to keep an appointment, we require at least 24 hours notice of cancellation.
